Admission is determined by the applicant's readiness in comprehending the programs set by the school as well as his ability in responding to the school objectives and mission. The committee on Admission is composed of the Directress, Principal, SAC Coordinator, Registrar, Guidance Counselor and Psychometrician. They are responsible in selecting new students. Test results, interviews and post academic performance as shown by the credentials and recommendations of the principal and guidance Counselor or class adviser are the common bases for admission.
Admission Procedures
Proceed to the Student Assistance Center (SAC) for inquiry.
Pay the admission processing and testing fee at the Cashier’s Office.
Present the Official Receipt of the processing and testing fee to secure a copy of the application form and recommendation forms at the Student assistance Center.
Only application with correct and complete requirements will be processed.
Admission Requirements
Photocopy of each of the following:
Birth Certificate with Civil Registry Number
Baptismal Certificate
Report Card/Form 138 (grades must be up to the latest grading period)
Three (3) pieces 1”x1” recent I.D. picture
Two (2) Letters of Recommendation (from the student’s recent former school) using the Canossa form:
one (1) from the Principal and
one (1) from the Guidance Counselor or the Class Adviser
For High School Scholarship Applicants Only:
Photocopy of the latest Income Tax Return (ITR) or affidavit of non-filing of ITR
Scholarship Recommendation (use Canossa Form)
Examination permit (to be issued after accomplishing the testing requirements mentioned above)
Pencils and ball pens (to be brought during the scheduled entrance examination date)
